To the maximum extent permitted by law, Adhisaat shall not be liable for indirect/consequential damages, specific medical outcomes, or on-ground execution by the Adhisaat Partner beyond its facilitation role. Nothing in this Agreement excludes any liability that cannot be excluded under applicable law.
Payments may be adjusted based on duty completion, cancellation timing, or service conditions as per Adhisaat policies. Adhisaat is not responsible for disputes arising from off-platform arrangements.
All personal, medical, and patient-related information shall be used only for service-related purposes and processed in accordance with applicable data protection laws, including the Information Technology Act, 2000 and the Digital Personal Data Protection Act, 2023, where applicable. Updated terms may be communicated through the Adhisaat mobile application, website, email, SMS, WhatsApp, or other official communication channels. Any incident, medical emergency, patient safety event, complaint, dispute, or security concern must be reported to Adhisaat immediately or as soon as reasonably practicable. Failure to report may be treated as negligence/misconduct and result in disciplinary action or termination.
Final settlement is subject to completion of all assigned duties, clearance of pending complaints or disputes, and return of any Adhisaat property. Adhisaat may withhold payments against verified liabilities.
The Adhisaat Partner shall be solely responsible for all applicable taxes and statutory compliances relating to earnings received through Adhisaat.
In case of emergencies or unsafe working conditions, the Adhisaat Partner shall immediately inform Adhisaat and act within their professional training and applicable protocols.
The Adhisaat Partner shall not be entitled to salary, provident fund (PF), ESI, gratuity, bonus, paid leave, or other employee benefits unless expressly agreed in writing.
Neither party shall be liable for delays or failures caused by events beyond reasonable control, including natural disasters, pandemics, government restrictions, strikes, riots, or similar events.

- Medical complications, deterioration of health, or death of the patient arising from the patient's medical condition or the independent professional judgment of healthcare providers;
- Any act, omission, negligence, or professional decision of independent healthcare professionals;
- Delays, interruptions, or inability to provide services due to traffic, emergencies, natural disasters, government restrictions, or other circumstances beyond reasonable control.
Nothing in this Consent Form excludes any liability that cannot legally be excluded under applicable law.
If an adverse medical event or the death of a patient occurs at the service location during or after the provision of services, Adhisaat shall not be liable, the underlying medical outcome solely by reason of arranging or coordinating the service, except where liability cannot legally be excluded under applicable law. All legal, medical, and administrative procedures thereafter remain the responsibility of the patient's family or Registered User.
The Registered User agrees to operate medical equipment only in accordance with the manufacturer's instructions and any guidance provided by Adhisaat or the healthcare professional. The Registered User shall inform Adhisaat in advance if any person at the service location has a communicable disease or if any condition may affect the safety of the healthcare professional.
The parties shall first attempt to resolve any dispute amicably through Adhisaat's grievance redressal process. If the dispute remains unresolved, it shall be subject to the exclusive jurisdiction of courts in Jaipur, Rajasthan, India.
I agree not to make false complaints, harassment claims, or misuse police/legal mechanisms against Adhisaat or its staff. Adhisaat may pursue legal action in case of false allegations or defamation. Nothing in this clause limits the Registered User's right to make genuine complaints or seek remedies available under applicable law.
Home healthcare involves inherent medical and environmental risks. Home environments do not provide the same infrastructure, monitoring, or emergency response as hospitals. By accepting services, I voluntarily assume these risks. The Registered User understands that home healthcare services have inherent limitations and are not equivalent to treatment provided in a hospital, intensive care unit, or emergency department. Certain medical conditions may require hospitalization, and the healthcare professional may recommend immediate referral to a hospital whenever clinically necessary.
Adhisaat services are not a substitute for emergency medical care. In case of a medical emergency, I accept responsibility for arranging ambulance services, hospital transfer, or emergency treatment. The Registered User acknowledges that Adhisaat does not operate an emergency medical response service unless specifically arranged.

- Personal data shall be processed in accordance with applicable data protection laws, including the Information Technology Act, 2000 and the Digital Personal Data Protection Act, 2023, where applicable.
- Personal data shall be retained only for the period required under applicable law or for legitimate healthcare, operational, regulatory, and legal purposes.
- Reasonable administrative, technical, and organizational safeguards shall be implemented to protect personal data against unauthorized access, disclosure, alteration, or destruction.
- Adhisaat shall make reasonable efforts to notify affected individuals where a personal data breach is required to be reported under applicable law.
- Where required under applicable law, personal data may also be disclosed to competent governmental, judicial, regulatory, or law enforcement authorities.
Adhisaat may update this Consent Form from time to time. Continued use of Adhisaat services after notification shall constitute acceptance of the revised terms.
This Consent Form shall be governed by the laws of India and subject to the exclusive jurisdiction of the competent courts at Jaipur, Rajasthan.
If any provision of this Agreement is held to be invalid, illegal, or unenforceable by a court or competent authority, the remaining provisions shall continue in full force and effect.
This Patient Consent Form constitutes the entire understanding between the Registered User and Adhisaat regarding the booking and delivery of services and supersedes all prior discussions, communications, or understandings relating to the same subject matter.
Adhisaat shall make reasonable efforts to arrange healthcare professionals according to the requested schedule. However, service availability is subject to the availability of empanelled healthcare professionals, geographic coverage, clinical suitability, and operational constraints. Adhisaat and the assigned healthcare professional reserve the right to refuse, suspend, postpone, or discontinue any service any service where the environment is considered unsafe, unlawful, abusive, or where continuation of the service may endanger the safety of the patient or the healthcare professional.
The Registered User authorizes Adhisaat to contact them through telephone calls, SMS, WhatsApp, email, mobile application notifications, or other electronic communication channels for service coordination, appointment reminders, billing, support, quality assurance, and other service-related communications.
Clauses relating to confidentiality, privacy, payment obligations, dispute resolution, governing law, indemnity, limitation of liability, and any other provisions intended by their nature to survive shall continue to remain effective even after completion or termination of the services.
The Registered User acknowledges and consents that the assigned healthcare professional may prepare and maintain treatment records, nursing notes, medication administration records, vital signs charts, photographs of wounds (where clinically necessary and with appropriate consent), and other clinical documentation solely for continuity of care, quality assurance, medico-legal purposes, insurance requirements, and compliance with applicable laws.
Unless specifically included in the booked service package, medicines, injections, vaccines, dressings, laboratory charges, consumables, disposables, and other medical supplies shall be arranged and paid for by the Registered User. Adhisaat shall not be responsible for the availability or procurement of medicines unless expressly agreed.
The assigned healthcare professional reserves the right to refuse or discontinue any procedure where:
- The required medical prescription is unavailable;
- The procedure is clinically unsafe;
- Necessary equipment or sterile conditions are unavailable;
- The patient's condition requires immediate hospital-based treatment; or
- Continuing the procedure may endanger the patient or healthcare professional.
The Registered User agrees to maintain reasonable hygiene and infection control measures at the service location, including availability of clean water, adequate lighting, electricity (where required), and reasonable sanitation to facilitate safe delivery of healthcare services.
The Registered User authorizes Adhisaat and the assigned healthcare professional, where necessary, to communicate with the patient's treating physician, hospital, laboratory, pharmacy, diagnostic centre, or authorized family members solely for coordination of healthcare services and continuity of patient care.
